At Touchette Regional Hospital (TRH), we are committed to protecting the privacy and confidentiality of your patient health information. 

Each person seeking services at Touchette Regional Hospital is given a TRH Notice of Privacy Practices that describes how the hospital can legally use and share your protected health information.

Our organization is committed to keeping your patient information private and secure. In order to continue keeping your protected health information secure, at TRH, we require photo identification and a signed patient authorization for releasing your protected health information. Click here for Patient Authorization.

If you do not have photo identification, we can take your picture for the authorization of releasing medical information. Once receiving your authorization and photo identification then your request will be processed and the protected health information will be submitted to the requestor. If your protected health information is being released to someone other than yourself, we will mail the protected health information to the recipient that you have specified or you can deliver the information. Please make sure that when you are requesting this information to go to a different individual other than yourself that you have the name, address, and phone number of this individual, physician, attorney and/or hospital.

Any correspondence is mailed normally within 30 days of receipt of the request.

Birth, marriage and/or death certificates

Touchette Regional Health Information department does not release any type of certified birth or death certificates. These can be obtained through the Illinois Department of Public Health, which has an online website for obtaining any certified certificate that you need. This website also gives options to obtain the certificates of U.S. citizens born abroad if needed: http://www.vitalchek.com/illinois-express-birth-certificates.aspx. A certificate can be also be obtained through the St. Clair County courthouse in Belleville, Illinois at the County Clerk's office on the second floor or on their website: www.stclaircounty.org/Offices/clerk.

Health Information Department

Requests for protected health information can be in the form of a fax, can be mailed or you can directly visit our department. If requesting by fax, please complete the patient authorization and provide a callback phone number. Then, fax your authorization and a copy of your photo identification card to TRH Health Information Department, attention, Correspondence at 618.332.5202. All mailed correspondence should be mailed to Touchette Regional Hospital, Attn: Health Information, 5900 Bond Avenue, Centreville, Illinois 62207. If you prefer to stop by the Health Information office, we are located on the first floor of Touchette Regional Hospital. Our hours are 8:00 a.m. - 3:30 p.m., Monday through Friday. The department is closed on holidays.
