Improving Quality of Life and Aftercare for Seniors

May 19, 2022

As May is Older Americans Month it’s the perfect time to focus on the importance of our health as we age.  Growing old is a natural progression in life, and most of us will need assistance at some point – due to injury, illness or just the natural regressions that occur as we get older, but that shouldn’t stop us from improving our quality of life.

Did you know Touchette offers a senior health program that focuses on improving quality of life for seniors after a hospital stay?

Senior I.Q. (Improving Quality) Program:

This program is a coordinated and community-based approach to improve the quality of life for seniors 60 and older. We unite physicians, community agencies, home health care staff, social workers, family members, and volunteers who work together as a team for our clients.

The overall goal of the Seniors I.Q. Program is to provide free aftercare services to the elderly to promote the best quality of life. Through this program, seniors continue living independently in their own homes and participating in the daily activities they enjoy. These seniors experience greater life satisfaction, which helps reduce the stressors associated with aging.

Could you benefit from getting assistance?

  • Developing individualized, holistic plans of care that improve physical, spiritual and social well-being
  • Performing home assessments that reduce safety risks at home
  • Connecting with Medicare/Medicaid and other healthcare services
  • Finding resources and services available within local communities (like assistance with utility bills or rent/mortgage assistance)
  • Receiving monthly food boxes to help meet nutritional needs

Let us help you live confident and independently with our senior aftercare services and education program.  At Touchette we pride ourselves in providing quality healthcare and education that focuses on the aspects of healing and recovery not only in the hospital, but in the comfort of your own home.
